CCF Privacy Policy for 360° Feedback

We, at Christ’s Commission Fellowship (“CCF”) acknowledge and respect your right to privacy and the confidentiality of your personal data. As we collect, use, store, disclose, and dispose the information you provide to us, we are committed to be faithful stewards of this information by treating it with utmost care and respect, protecting your personal data, and respecting your privacy.

 

While designing and executing our 360° Feedback Form, it is within our Privacy Policy to take all necessary steps to ensure that the personal data you provide is processed securely and lawfully.

 

Only authorized personnel obliged to respect its confidentiality has access to personal data. We neither sell, rent, or exchange any personal information supplied by you to any third party, nor use any of the information you provide for commercial or unrelated purposes.

 

It is within our Policy to regularly monitor our internal procedures to ensure compliance with the relevant statutory requirements, including the general policies and procedures of Republic Act No. 10173 or the Data Privacy Act of 2012 of the Philippines (the “Data Privacy Act”), relevant orders issued by the National Privacy Commission, and other applicable laws, rules, and regulations.

 

The Types of Information We Process

The 360° Feedback is meant to get feedback from family members, discipleship group (“Dgroup”) upline, Dgroup downline, fellow Dgroup members, and other persons with whom the participant is sufficiently acquainted with. The participant shall be evaluated on questions that may entail disclosure of personal or sensitive personal information of the participant, such as, but not limited to:

 

  • Name, age, gender, contact information
  • Marital status
  • Financial situation
  • Medical or information about health
  • Religious convictions
  • Sexual life
  • Legal proceedings or disciplinary cases

 

As the 360° Feedback has open-ended questions, we have no control or supervision on the kind and type of information that will be provided by your evaluators. Due this fact, CCF does not warrant or guarantee the accuracy or completeness of information provided by the evaluators through the 360° Feedback Form. Information provided therein are meant for further discussion with the participant by their pastor or Dgroup Leader during discipling or accountability sessions.

 

Cognizant of this limitation, CCF shall not act or make decisions that would affect the participant based solely on anonymous or unverified information.

 

Why we collect Personal Information from you

In helping our Dgroup leaders towards Christlikeness, CCF has implemented the 360° Feedback as a tool in evaluating and mentoring Dgroup Leaders with the goal of instilling Christlikeness in their various areas of life.

 

The 360° Feedback is our church practice of eliciting feedback from the family members and Dgroup members of the Dgroup Leader – for the purpose of discipling and accountability, as well us to understand the issues that are commonly faced by our Dgroup Leaders and develop programs, sermons, and solutions to help address such issues.

 

How we collect Personal Data

We collect personal data of the participant upon the voluntarily submission of the same to us through the 360° Feedback Form.

 

How we use your Personal Data

We guarantee that the information you have voluntarily provided will be used solely for the aforementioned purposes for which it was collected, will be kept confidential, and will not be disclosed without your consent, except for lawful reasons as provided under the Data Privacy Act (i.e. lawful order by a competent government authority).

 

Who we share your Personal Data with

Your personal data may be disclosed to authorized staff and pastors provided that CCF will only share the necessary information needed to deliver specific objectives or purpose of the 360° Feedback. Also, for purposes of accountability and discipling, information provided about the participant from the evaluators may also be disclosed to the Dgroup Leader of the participant.

 

We may also disclose to the proper officials or authorities if required by law or by a lawful order.

 

We will never share or disclose your information for commercial or non-related purposes.

 

How we protect your Personal Data

Reasonable steps have been set up to ensure the security of personal and sensitive information we obtain including the installation of organizational, physical, and technological safeguards for as long as they remain in our digital storage until such time they are no longer needed, but not exceeding ten (10) years from the date that said information is obtained. Additional steps will be taken to destroy or de-identify personal information that is no longer required.

 

However, we disclaim any responsibility for any breach to the systems of technological companies whose technology runs our system or hosts our cloud database that is due to a systemic cybersecurity attack against such companies and not due to the fault or negligence of our employees.

Accessing of Personal Data

A participant may request for a copy of the information submitted through the 360° Feedback Form. However, as there may not be sufficient manpower to handle all requests, kindly give our team at least thirty (30) working days after the closure of the online link to process your request.

 

Updating your Personal Data

Any changes on the Personal Information provided will only be made upon the receipt of the updates therein by CCF and/or upon the request thereof by the primary data subject. Upon update thereof, both the new and the retracted information and the simultaneous receipt of the new and the retracted information shall be made accessible to the recipients thereof. Authorized personnel and third-party providers who have previously received such processed personal information shall  be informed of its inaccuracy and its rectification upon reasonable request of the data subject.
